York Lodge, Horley, Surrey
York Lodge is an eight-bedded detached house close to Horley Town Centre. The home is for adults with a mild to moderate learning disability, who may also have some associated support needs such as enduring mental ill health. All tenants are encouraged to participate actively in the day to day running of the home.
There is a focus on being person centred and individuality such as everyone doing their own shopping and preparing their own meals. New referrals should be made with regard to the fact that the home will be converting to a supported living service.
2 Coombe Oak, Warren Road, Kingston upon Thames, KT2 7HY
2 Coombe Oak is situated in a private residential road in Kingston. It is a bungalow with all accommodation on the ground floor. It is a short walk from local shops, Kingston town centre can be accessed by public transport and Richmond Park is very close. .
2 Coombe Oak is home to 5 adults who have profound learning disabilities. All the service users need a high level of care and support in all aspects of their lives. The aim of the service is to enable the service users to participate within the community, and encourage informed choice.
Each service user has their own support Key Worker who is responsible for ensuring that the Person Centred Plan is implemented, maintained and reviewed on a regular basis. If tenants are unable to communicate verbally, other methods of communication are used (i.e. Makaton, facial expression).
