CURRENT VACANCIES – SUPPORTED LIVING Welmede Housing Association currently has vacancies at the following Supported Living scheme located in Surrey. Elm House is a 7 bedded unit for young people in transition. Referrals are taken for young people with a learning disability aged between 18 yrs and 25 yrs. The young person may stay for up to but no longer than 2 years. Placements are funded via the Supporting People programme. tenancies are fixed term. Each young person will have 25hrs support provided each week to encourage and support them to further independence. The person MUST be on the housing register for their own borough and there must be an agreement for them to return after the 2 years. No sleep-in staff are provided so young persons must be safe to leave overnight. The house is a detached 9 bedroom house with a large living/dining area, conservatory, kitchen, utility room and room .One bedroom is used as a quiet area if the young person wishes some other space. There are three bathrooms with toilets and one separate toilet. Off street parking is available. Young people are assisted with college/work/leisure and social activities. The young people are encouraged to take part in decision making within the house. Rectory Lane is a semi detached house in Byfleet . Well maintained front and large back garden, communal; Lounge, kitchen, dining room, bathroom and separate WC. The house is walking distance to local shops, and facilities. There are currently 2 tenants, 1 male and 1 female, living in the home who have a mental health need but can function without on site staff. Supporting People funds attached of up to 3 hours Newark Court, Walton-on-Thames Newark Court is a modern three-bedroom mid terrace house situated on an estate about ¾ mile from Walton town centre where there is access to all the necessary amenities. It has a good sized kitchen dinning room with access to a medium sized well tended back garden, lounge, downstairs cloakroom, three good sized bedrooms and bathroom on the first floor. This shared house would be suitable for a 2 males, with a learning disability, over the age of 25. Supporting People funds attached are 9 hours and 12.5 hours. Laburnum Road, Woking, Surrey
Laburnum Road is a two storey, three bedroom maisonette. There are currently two male service users living in the home who have mental health needs but can function without on site staff. Supporting People Funds attached of up to 3 hours
Hammond Road is a semi detached, 3 bedroom house situated in a quiet Road, 5 minutes walk to local shops and public transport. There are currently two male tenants with a learning disability living in this home that are able to live without on site staff. Supporting People funds attached of up to 14 ½ hours. Nursery Road, Knaphill, Woking Nursery Road is a three bedroom end of terrace house in Knaphill. Well maintained front and back garden, communal; Lounge, kitchen, dining room, bathroom and downstairs WC. The house is walking distance to local shops, and facilities. There is currently 1 male and 1 female tenant living in the home who have a mental health need but can function without on site staff. Supporting People funds attached of up to 3 hours
Church House Church House is a large 5 bedroom house in Sheerwater Front drive with off street parking for 4 cars, back garden, communal lounge / diner, kitchen, downstairs WC, upstairs bathroom and WC. There is currently I female and 3 male tenants with mental health needs living in this property ages between 54 and 83. Supporting People funds attached of up to 12 hours
