Young Transition

Our Young Transition Programme supports people with learning and physical disabilities on the journey from leaving home, school or college into the next chapter of their life.

We work with the young person, families and professionals to develop a person centred plan (PCP) that supports their ambitions and dreams for the future.This is an extremely exciting time for many people, but we also recognise it is a period of change for the young person and their family. This may impact emotionally as well as practically and present some challenges and concerns for those involved. We provide support and flexible programmes that reflect these issues to balance the need for independence, choices and rights with responsibility and keeping safe.

Welmede provide accommodation that reflects young people’s lifestyles focusing on the need for independence and privacy as well as structure, social interaction and support. The young person will need to feel it is their home and be involved in all aspects of it.

People are supported to enhance existing skills as well as develop independence in other areas of their lives. Through the development of the PCP we work with individuals to assess their needs and wishes and design a programme to suit. This could be around financial management, life skills, keeping safe, assertiveness, managing their tenancy and so on.

We employ many young support workers who have similar interests to those involved in transition. This often supports confidence building as there are shared activities and experiences. Support staff assist the young person to develop links with the local community which may include employment opportunities, further education or accessing local activities and facilities, shopping and travel. Key-workers support each person to develop their PCP, support plan and transition programme.

As part of the transition individuals will be supported to look at planning their future including housing and support needs.
